HS-USA : HS-USA Patient Surveys

Thank you for checking in with HS-USA Surveys!  If you are afflicted with this disease we want to hear from you!  Please read the points below before choosing a survey from the list.

• Take the time to answer as many questions as you are able to answer.  However, all questions are optional, so you may skip a question if you don't have an answer for it.
• Once you start a survey you'll have to finish it.  The system will not save a half-completed survey for you. If you leave, you'll have to start over next time. Do not submit a half-completed survey, since you will not be able to come back to it. 
  All surveys are categorized and short enough to be completed in under 10 minutes.
• You will see the accumulated results (including yours) as soon as you submit the survey.  You can always come back to this page and click the Results column in the list below to view each survey's results.
• HS-USA surveys are completely anonymous.  We collect absolutely no information that may identify you as an individual. We do collect your IP (computer) address to ensure that surveys are not submitted multiple times.
• If you are sharing your computer with another sufferer of HS who would like to submit a survey, please send an email to webmaster@hs-usa.org - we can "tweak" the software on a one-off basis to let you do that.
• Tell others!  Pass on the web address for this page to anyone you know who has this disease.  We should be able to get at least 500 respondents to each survey!  Please help us make that goal happen!
